Make a gift of securities to LVT
Your broker can transfer securities to LVT through the following account with the Community Foundation for Southern Arizona. Follow these easy steps:
- Call the office for a Authorization for Transfer form. Give it to your broker with the following information.

- Do not sell the stock prior to transfer.
- Notify LVT (882-8006) AND the Community Foundation for Southern Arizona (770-0800) no later than the day you give these instructions to your broker so we can monitor and track your stock donation to assure its timely delivery.
- Both LVT and the Community Foundation will need the following information: your name, phone number, stock name and number of shares.
