Tutor Forms and Required Reports

To maintain an overview of tutoring and student progress, we require that you report total tutoring hours and total preparation and transportation time to the LVT Tutor Center Supervisor or directly to the LVT office by mail, e-mail, or fax at the end of each month. We have four forms: Basic Literacy long form, Basic Literacy short form, ELAA Center and ELAA One-on-One. We are eager to learn about student progress and any goals achieved. Please report these on the monthly form, as well. If you stop tutoring, please let us know at 882-8006. (The reports are also required by ProLiteracy America.)

The forms included on this page require Adobe Acrobat Reader. MacIntosh users, you will want to use Acrobat Reader instead of Preview if you wish to take advantage of filling out the forms and electronically. You may obtain the latest version of the reader by clicking here.

These forms can be completed electronically and emailed to LVT or printed and faxed or mailed to LVT.
